Provide telehealth consultations for follow-ups, medication adjustments, and urgent needs
Diagnose and manage chronic and acute conditions
Perform wound care, preventive screenings, and transitional care services
Develop and update treatment plans based on patient needs and goals
Document patient encounters accurately using our EHR system
Coordinate care with specialists, home health, hospice, and family caregivers
Maintain communication and follow-up via virtual platforms when needed
